Performance Evaluation Information Performance Evaluations for the FY08 evaluation period. The dates covered in the period are March 16, 2007 - March 15, 2008 for Unit 1, classified employees, faculty members and professional staff. Evaluations are due in Human Resources by Wednesday, May 7, 2008 Evaluation Forms (Network login using your email id and password is required when accessing the forms from an off-campus location or public access computer on-campus.) Important Notes: Classified evaluations cannot be started until after March 15, 2008. All drop-down choice fields contain the word "Select" to indicate that a choice must be made. When you open the form a download window may appear, you should choose to "Open" the file on your computer. Once open, you may save form just as you would a word document. We recommend that you that you use the employee's name as the file name. Microsoft Office 2007 Enterprise is required to use the forms available on the web. The software is available for employee purchase at the campus bookstores. Fill-in Word forms are available in the Human Resources community on the InnerLoop. When accessing the forms from an off-campus location or public access computer on-campus, you must login using your e-mail ID and password to access the forms.
