How to make a report
Help us by making the following referrals when necessary:

Academic Dishonesty Incident

Faculty – this form is designed just for you. Students that are being warned or sanctioned for Academic Dishonesty should be reported using this form.

CCBC CAREs Outreach and Concern

If you know of a student that needs assistance or may be in crisis, please use this form to refer them to our CARE team and College and Community Outreach Success Navigators.

Sexual Misconduct Incident

CCBC faculty and staff are mandated reporters for sexual misconduct. Please use this form to reach our Title IX Coordinator.

Student Conduct Incident

If you believe that a student may have violated the code of conduct, or that their behavior could escalate to a code of conduct violation, please use this reporting form.

Please be assured that:
  • All reports are confidential.
  • You will receive acknowledgement that the report has been received and that appropriate action will be taken.

What are the advantages of filing a report?
  • Student behaviors will be tracked. A behavior occurring in your class or any place on campus may be occurring in other classes or in other locations on campus.
  • Concerning behaviors can be identified. These are behaviors that are inappropriate or unusual.
  • Immediate threats will result in immediate action.
  • A distinction can be made between a behavior that warrants immediate attention and one that requires an intervention plan for the student.
Your support of this process will result in a more civil environment in which learning can take place. Also, the expectation of appropriate behaviors by students in this learning environment will be reinforced.
