According to the Americans with Disabilities Act of 1990, the term "disability" means, with respect to an individual

  1. a physical or mental impairment that substantially limits one or more of the major life activities of such an individual;

  2. a record of such an impairment; or

  3. being regarded as having such an impairment.

In order to most effectively meet the needs of its students with disabilities, the Disability Support Services Office at CCBC is requesting that students with disabilities complete this form. Completion of this form is purely voluntary.

Disability Support Services provides academic accommodations, counseling and advocacy for students with disabilities. If you have any special needs related to your disability, we urge you to contact Disability Support Services. Documentation of disability will be required for accommodations. Please contact the DSS office on the campus that you wish to attend for more information.

Any information given to Disability Support Services is confidential and will only be released to other offices on campus with your permission. Information concerning disability will not be indicated on a transcript or any other official document.
